A company is looking for a Social Worker - LCSW in New York.
Key Responsibilities
Provide integrated medical, behavioral, and social care to patients
with complex health care needs
Deliver brief evidence-based treatment for mental health issues and
support psychotropic medication management
Document patient encounters and facilitate communication regarding
patients' behavioral health needs across the clinical team
Required Qualifications
Master's degree in Social Work
Current New York LCSW required
2+ years of prior healthcare/behavioral health experience
required
Geriatric care experience highly desired
Proficient in computer skills, including Microsoft Office Suite and
EMR experience
